CDC Issues Updated Guidelines on Preventing Perinatal Group B Strep Transmission

The CDC has updated its 2002 guidelines on the prevention of group B streptococcus (GBS) transmission to infants during labor. The recommendations appear in MMWR.

Among the changes:

  • There are now two separate algorithms for GBS prophylaxis during possible preterm labor — one for spontaneous preterm labor and another for premature rupture of the membranes.
  • If preterm labor is ruled out, GBS prophylaxis should be discontinued.
  • While penicillin remains the treatment of choice for prophylaxis, penicillin-allergic women at high risk for anaphylaxis should receive clindamycin or vancomycin, depending on the susceptibility of their GBS isolate; erythromycin is no longer acceptable.
